Exercise PolyBase::convex_hull_assign_and_minimize() with empty
polyhedra in all possible ways (exposes more bugs in the current
PolyBase implementation).

Corrected bugs in methods insert(g), add_generators(gs) and
add_generators_and_minimize(gs): in the case of NNC topology,
we were not adding the closure points corresponding to each added point.
