Log message: Simplified function prepare_for_1st_ph_simplex(), avoiding useless copies and factoring 2nd and 3rd Row& arguments into a single Matrix& argument. Adding slack variables all in one shot, instead of one at a time. Simplified function first_phase() by avoiding useless matrix copies.
